Dear Parent or Guardian, With your permission, Immanuel Baptist Church would be honored to provide transportation for your child(ren) to our Vacation Bible School, Awana Clubs, or other church-sponsored activities for children. In order to do this responsibly and safely, we ask for your consent . Please read and sign the statement below. Please have this form filled out before your child(ren) before boarding. This signed permission form will expire January 1, 2019. Thank you!
Parent/Guardian Statement:
I give my consent for my child(ren) named above to ride to and from Immanuel Baptist Church events and activities on the Immanuel Baptist Church bus. Further, I agree to hold Immanuel Baptist Church, it's officers, workers, volunteers and representatives guiltless in the event of an accident or injury to my child(ren). By agreeing to this statement and filling out this online bus rider waiver form, I affirm that Immanuel Baptist Church has the authority to act on my behalf in the event of an accident or injury to my child(ren) provided reasonable effort has been made to contact me. I also affirm that each child listed is over 8 years old or 80 pounds in accordance with the North Carolina Passenger Safety Law.
